Our Office Information

Dr. Jeffrey Wallis and his staff are pleased to have this opportunity to provide you with the best possible gastrointestinal care available. Your health, comfort and convenience are foremost in our minds. Dr. Wallis is a gastroenterologist and hepatologist who specializes in the care of diseases and disorders of the gastrointestinal tract, including the stomach, liver and pancreas. Insurance Information

Insurance/Fees

As a courtesy to our patients, Dr. Wallis participates with Medicare, as well as most of the regional health insurance carriers. The patient will be responsible for the co-payment and any deductible at the time of service. For your convenience, we file all primary health insurance claims on behalf of our patients. In order to provide care to multiple patients, we expect in turn to be paid promptly and in no case in 30 days. Any disputed claims are clarified, reviewed and rectified per company policy. Your 3rd. party payor's prompt response is appreciated and required.

 

Insurance Cards

Please bring your insurance cards. We will make a copy of them, so we may provide your insurance company with any information they require to process your claims as efficiently and accurately as possible. We emphasize patient education and encourage you to ask any questions that you may have regarding your health care.

Presently, we are contracted with Medicare, Medicaid, Blue Cross/Blue Shield, Cigna, Aetna, United Healthcare, Avmed, Beechstreet, Coventry, Freedom Health, GHI, Multiplan, Quality Health Plans, Railroad MCR, Tricare For Life, Evolutions, Secure Horizons, Medicare Advantage, Medipass, First Health, Evercare , Universal Healthcare, Medco, Humana, VA, Healthease, Optimum, Humana Medicare HMO, United Medicaid/Community Health Plans and Wellcare of Florida.

We require authorization for both office visits and procedures for the following: VA, Blue Cross/Blue Shield of Florida Health Options Inc., Freedom Health, Humana HMO, Humana Medicare HMO, Quality Health Plans, Universal Healthcare, Optimum, Secure Horizons, Wellcare, Healthease, United Healthcare, Avmed, Medicaid, United Medicaid/Community Health Plans & Medipass. Other insurance carriers are handled on a case by case basis, so please inquire when scheduling your appointment. There are also some specific procedures and patient circumstances which are required to be performed in a hospital setting.

 

 

Office & Appointment Policies

Appointments

Our office hours are 9:00 a.m. to 5:00 p.m. Monday through Friday. We will be happy to schedule an appointment time most convenient for you during our normal office hours. We will make every effort to honor all time commitments and request that you extend the same courtesy. Please notify us immediately if you are unable to keep your scheduled appointment, so we may be of service to others.

 

Emergencies

 Dr. Wallis may be contacted 24 hours a day for any emergency situation. If an emergency occurs after hours or on weekends, please call 352.564.3900. Our answering service will contact Dr. Wallis or the physician on call for our practice. However in a dire emergency, dial 911 or go immediately to the nearest hospital emergency room where evaluation and treatment may begin.

 

Your First Visit

Dr. Wallis will use his specialized training and experience to give you an in-depth consultation to determine your diagnosis and treatment.

MEDICATIONS: We request that you bring a list of your current medications and dosages, as well as any allergies you may have to medications.

FORMS: You can print patient registration and health history forms from our website. These forms are required to be completed at the time of your first visit. Click Here to visit our Patient Form Downloads.

RECORDS: To facilitate timely care, please obtain any pertinent medical records and bring them with you to your first visit. All of this information will enable Dr. Wallis to provide you with proper evaluation and treatment.
